student registration
PLEASE READ ALL INSTRUCTIONS AND STATEMENTS CAREFULLY.
Although not required, it is strongly recommended that you tour the school before submitting a registration form.
forms to complete and sign
The following four forms are REQUIRED for admission to IPR:
New students and new incoming siblings of IPR students must complete and submit all four forms.
Returning IPR Students
If you are completing registration for a returning IPR student and you have completed these forms in the previous 1 or 2 years, you do NOT need to complete new ones. You will be asked to review your forms and verify that all information is still correct. However, if you know you have substantial changes, please complete a new form.

- Pick-Up and Emergency Contact
- Allergy/Diet/Health - even if your child has no allergies or health concerns, you must complete the form by indicating none in each section.
- Medical Consent and Waiver
- After School Program Release - even if your child is not currently signed up for an after school program, we require that this be signed and returned at the beginning of the school year. This ensures that it is in place should you decide at any time during the year to sign your child up for a program.

These forms may be completed and uploaded (at the bottom of this page) with your electronic registration submission. Alternatively, you may print these forms, complete them, and bring them with you to the front office when completing your registration.

In addition to the forms above, an original copy of the School Policies & Procedures must be printed, initialed, signed, and submitted in person to the front office. This document will be available for you to print and complete once you hit the Submit button below.
